Richland County restaurant inspections: critical violations Aug. 22

The following restaurant inspections with critical violations were conducted by Richland Public Health between Aug. 22 and Aug. 24:

Lex Food Mart, 98 Lexington Ave., Mansfield, Aug. 22. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed packages of raw bacon on a shelf above ready-to-eat foods in the reach-in cooler. PIC removed bacon from sale. TCS foods not being cold held at the proper temperature (critical, corrected). A package of hot dogs in the reach-in cooler internal temperature 48 degrees. PIC removed hot dogs and lunch meat from sale.

Red Fox Tavern Inc., 301 Marion Ave., Mansfield, Aug. 22. Person in charge (PIC) unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC could not state the correct concentration for sanitizing solution (200-400 ppm). Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed uncovered ice in the ice bin at the bar.

Subway #12061, 950 Ashland Road, Mansfield, Aug. 22. PIC unable to demonstrate knowledge by having no critical violations (critical). Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical). No quat solution at three-compartment sink to sanitize dishes. Correct by Aug. 23. Presence of live insects, rodents and other pests (critical). Observed fly and gnats in kitchen/lobby area of facility. Correct by Aug. 23.

Raintree, 721 Scholl Road, Mansfield, Aug. 23. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed no quat solution at three-compartment sink.

Happy House Inc., 201 #3 E. Main St., Lexington, Aug. 23. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC stated that utensils not held at temperature are only cleaned once a day (more than required every 4 hours). Also observed PIC washing utensil in prep sink without ability to properly sanitize. Education provided. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected). PIC stated that foods were being left out at room temperature to cool until about 70 degrees then being put into the walk-in cooler and cooled. PIC also stated that temperatures are not being taken during this process. Food employee(s) not washing hands in approved handwashing sink or automatic handwashing facility (critical, corrected). Observed PIC washing hands in the prep sink. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed raw chicken stored over diced vegetables in broth. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed the following foods to be uncovered: sliced carrot, onion, iced veggies in broth, and bamboo shoots. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed blade of can opener to have visible residue. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed food contact portion of the meat grinder that was stored clean to have visible dried residue on inside lip of the blade funnel. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed the food contact portion of the following pieces of equipment stored in clean dish areas to have visible residue: sheet pans, glass containers, utensils, plastic containers and knives. TCS foods not being cold held at the proper temperature (critical, corrected). Observed house made garlic and oil sitting on a cart without temperature control to have an internal temperature of 71 degrees. Ensure garlic in oil is stored with temperature 41 degrees or below. PIC voluntarily discarded. Improper storage of poisonous or toxic materials (critical repeat, corrected). Observed aerosol can of gas for torch being stored over bags of rice. Presence of live insects (critical repeat). Observed small black flying insects throughout the facility, specifically notating near the prep sinks, warewashing area, under the Pepsi dispenser machine in the lobby, and the back storage area near the hot water tank. Remove all evidence of insects/ contact a licensed pest control operator by re-inspection Aug. 30. Food contact surfaces not easily cleanable (critical repeat, corrected). Observed strainer with product being stored in the walk-in sitting on top of an cardboard egg carton. rendering the food contact surface no longer smooth, durable, non-absorbent, and easily cleanable. Food contact surfaces not easily cleanable (critical, corrected). Observed paper towels and cardboards being used as a food contact surface for fish and egg rolls.

Circle K #4705705, 30 E. Cook Road, Mansfield, Aug. 24. PIC unable to demonstrate knowledge by having no critical violations (critical). Observed two or more critical violations; failure to properly maintain facility. TCS foods not being cold held at the proper temperature (critical). Observed back-room True cooler that was reading 45 degrees. Observed cold-brewed coffee holding unit reading 52 degrees. Unit was put out of order during inspection. Correct by Aug. 28. Improper storage of poisonous or toxic materials (critical). Observed various cleaning agent bottles stored above produce wash sink. Correct by Aug. 28. Presence of live insects, rodents and other pests (critical). Observed gnats around and inside donut case. Employee was asked to dispose of donuts that are unprotected from insect contact. Correct by Aug. 28.

Winchester Terrace Nursing Home, 70 Winchester Road, Mansifeld, Aug. 24. Water source and/or hot water capacity not sufficient (critical, corrected during inspection). Hot water at the three-compartment sink reached 107 degrees. Discussed adding hot water to bring wash water up to at least 110 degrees until repaired.

Steve's Dakota Grill, 3101 Park Avenue West, Mansfield, Aug. 24. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed uncovered onions in the walk-in, rendering them exposed to sneeze, splash or dust. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed packaged raw animal foods stored over salad dressings in the walk-in cooler. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Observed chlorine sanitizing machine to have a concentration below 50 ppm. Equipment and/or utensils improper construction (critical, corrected). Observed 2 pounds of rolls in the walk-in cooler to be stored in direct contact a non-food grade to-go plastic bag. PIC voluntarily discarded at time of inspection. Insufficient air gap between the flood rim and the water supply inlet (critical repeat, corrected). Observed no air gap in the under the bulk ice machine.
